Acceptable Identification for USPS 1583 Form
The U.S.Postal Service (USPS) requires all Commercial Mail Receiving Agents (CMRA), like Earth Class Mail, a LegalZoom Company, to collect a notarized USPS Form 1583 from customers. This form verifies your identity and provides official consent for us to receive and manage mail on your behalf.
Two forms of identification must be presented to our online notary partner during the notarization session. These must include a primary ID with a photo, and a secondary ID with a home address.
Our notary partner speaks both English and Spanish. The notarization interview must be in one of those languages. Your ID must also be in a language that uses the Latin alphabet.
Primary Identification:
Your primary identification card must be one of the following documents:
Non-US Passport This includes foreign passports not issued by the United States.
Matricula Consular (Mexico) A Matricula Consular card is an identification card issued by the Government of Mexico through its consulate offices to Mexican nationals residing outside Mexico.
NEXUS (Canada) NEXUS is a joint Canada Border Services Agency and U.S. Customs, and Border Protection operated trusted traveler and expedited border control program. Customers may provide a NEXUS card as a form of identification for money orders. To be accepted, the NEXUS card must contain an identification number.
Secondary Identification:
(Must include home address)

- Mortgage
- Deed of trust
- Voter Registration Card
- Vehicle Registration Card
- Home Insurance Policy
- Vehicle Insurance Policy (must have the policy number and the current policy period)
