The United States Postal Service (USPS) requires all Commercial Mail Receiving Agencies (CMRAs), like Earth Class Mail, a LegalZoom company, to collect a notarized USPS Form 1583 from customers. This form verifies your identity and authorizes us to receive and manage mail on your behalf.
To complete USPS Form 1583, you must provide two forms of valid identification during your notarization session:
- One Primary ID — must include your photo
- One Secondary ID — must include your current home address
These identification requirements are established by USPS and are necessary before mail processing for your virtual mailbox can begin.
*Please note that billing starts upon account creation and is not dependent on the completion or approval of USPS Form 1583.
Our notary partner currently supports notarization sessions in both English and Spanish. Please note that all identification documents must use the Latin alphabet so the information can be properly verified during the notarization process.
Important Requirements
IDs Must Be Valid and Current
All identification documents must:
- Be valid and unexpired
- Be government-issued (where applicable)
- Include clear, legible information
- Match the information entered on your USPS Form 1583
Expired, blurry, cropped, or damaged IDs may be rejected.
Names Must Match on Both IDs
The name listed on your primary and secondary identification documents must match exactly, including both first and last name consistency across both IDs.
If your IDs show different last names (for example, due to marriage, divorce, or a recent legal name change), your USPS Form 1583 submission may be rejected.
Please ensure both IDs reflect the same legal name before your notarization session.
If you have recently changed your name, we recommend updating your identification documents prior to completing the notarization process.
Acceptable Primary Identification
You may provide one of the following as your primary ID:
U.S. Government I.D. - may be federal, state, or tribal-issued. Customers may provide the following:
- State-issued driver’s license
- Passport - This includes a U.S. passport, U.S. passport card, or foreign passport
- Non-driver’s identification card

U.S. permanent resident or other identification card issued by U.S. Citizenship and Immigration Services
Note: We do not accept temporary identification documents (such as temporary driver's licenses or temporary state ID cards). Please provide a fully-issued, permanent ID.
Acceptable Secondary Identification
You may provide one of the following as your secondary ID:

- Driver's License (US-Issued)
- State ID Card (US-Issued)
- Lease
- Mortgage, or
- Deed of trust.

- Vehicle Registration Card
- Home Insurance Policy

Common Reasons IDs Are Rejected
Your identification documents may be rejected for any of the following reasons:
- IDs are expired
- Names do not match between documents
- Address information is incomplete or inconsistent
- Images are blurry, cropped, or unreadable
- Documents are not accepted by USPS
- IDs are not written using the Latin alphabet
- Information entered on Form 1583 does not match submitted IDs
